Pathology Department “Ag. Pavlos” Gen. Hospital Thessaloniki, Urology Department Gen. Hospital Halkidiki, Greece

A 78-year old diabetic man who presented with painful swelling of the right testis resistant to antibiotic therapy was subjected to orchiectomy. Malacoplakia of the testis was diagnosed based on the characteristic histologic findings of numerous von Hansemann histiocytic cellsand intracellular and extracellular Michaelis-Gutmann bodies. Testicular malacoplakia is very rare and the diagnosis is only made by histology. Compromised immune status, including neglected diabetes mellitus, may be associated to the pathogenesis of this lesion.
