A case of ductal adenoma of the breast with apocrine morphology, in a 51 year old woman, is presented. The lesion a circumscribed painful nodule of 2,5 cm accompanied by nipple discharge, was considered suspicious of malignancy at the clinical and laboratory preoperative investigation (mammography, fine needle aspiration). The same ambiguous feature was observed at frozen section examination. The real nature of the tumor as a benign neoplasm and its precise classification was apparent at the histologic study and by immunohistochemical findings. The correct identification of this unusual mammary neoplasm minimize the danger of confusion with carcinoma, avoiding of an inappropriate mastectomy.

Key words: Breast, ductal adenoma, apocrine change.
