Epithelial-Myoepithelial carcinoma of the parotid gland.

We report a case of epithelial-myoepithelial carcinoma of the parotid gland that occurred in a 82-year-old man. The patient presented with an asymptomatic, rightsided subauricular mass. Clinically, the tumor was localized, well defined, movable and had a size of "walnut". Superficial parotidectomy and pathological examination of the mass was perfomed. Histology showed a tumor characterized by wellcircumscribed, encapsulated nodules, composed of duct-like structures lining of inner ductal cells and outer clear myoepithelial cells. Nuclear atypia and mitotic activity were rare. Immunohistochemical examination revealed that the inner ductal cells were positive for cytokeratin 8, 18, 19 surrounded by SMA positive myoepithelial cells with clear cytoplasm. On the basis of the histological and immunohistochemical findings the diagnosis of epithelial-myoepithelial carcinoma was made. Epithelial-myoepithelial carcinoma is a rare tumor accounting for less than 1% of all salivary gland neoplasms. It is a neoplasm of low malignant potential and arises most commonly in the parotid gland. Epithelial-myoepithelial carcinoma presents a high incidence of recurrence and rarely lymph nodal and remote metastasis. Complete surgical resection is the best treatment and adjuvant radiotherapy may be effective in preventing local recurrence.
