Solid pseudopapillary tumors are uncommon neoplasms of low malignant potential and uncertain histogenesis usually occurring in young women. They usually present as intrapancreatic cystic-solid large tumors but parapancreatic sites are described. They are frequently asymptomatic and the neoplasm is characterized by slow growth, low metastatic potential and a long survival even with disseminated disease. This is a report of a case of a 22-year old female who presented with intermittent abdominal pain and vomiting. Computer tomography (CT) and magnetic resonance imaging (MRI) revealed a mass in the tail of the pancreas. En-block distal pancreatectomy and splenectomy was performed and a tumor measuring 15x13x8 cm was identified. The mass was encapsulated with extensive haemorrhage, mostly cystic, with friable, red brown, solid areas. The histological features and the immonophenotype were characteristic of a solid pseudopapillary tumor. In follow up the patient was found free of disease and alive three years later.
