Carcinosarcoma of the upper respiratory tract.
A case report

We report the case of a 52-year-old male patient, admitted in our hospital with a tumor in the left cervical area. Excision of the tumor and cervical lymphadenectomy were performed; the histological examination revealed a metastatic poorlydifferentiated squamous-cell carcinoma. The same patient had recurrences after 4 and 4.5 years at the base of the tongue. Histological and immunohistolochemical examination revealed a spindle-cell carcinosarcoma in both cases. Six months later, two recurrences emerged in the laryngeal epiglottal area and oral cavity, which led to total laryngectomy. Histological examination revealed a poorly-differentiated neoplasm with increased cellularity consisting of large spindle-cells with brisk mitotic activity; their immunophenotype was consistent with a neoplasm of mesenchymal origin. In the consecutive recurrences, the primary neoplasm was even less differentiated, whereas in the last one it underwent total mesenchymal metaplasis to smooth muscle cells.
