Differentiated sarcomatoid neoplasm of the urinary bladder

A case of a urinary bladder sarcomatoid tumour in a 69-year old man is reported. The tumor presented with gross hematuria and it was broad and exophytic, invading the right ureter orifice. Microscopically, the mesenchymal element dominated the malignant neoplastic tissue. An immunohistochemical analysis was performed. The tumour cells reacted positively with both epithelial and leiomyosarcomatous markers, confirming the dedifferentiated state of the neoplasm. The treatment of choice is radical cystectomy, perhaps preceded by radiation therapy.
