Patient with chronic disease. What is the diagnosis and what mistake has been made?

Multiple pseudopolyps along the whole length of the large intestine. This is the picture of toxic megacolon on an underlying ulcerative colitis. Barium enema and colonoscopy are contraindicated in cases of toxic megacolon. Plain X-ray of the abdomen, which as a rule precedes other investigations, provides the definite diagnosis.
